The purpose of the CoP is to provide a forum where colleagues with experience and responsibility in the subject area can:

• Meet together, compare learning and share good practice.

• Work together to create materials to help support and embed integrated models of care, and to share learning more widely with colleagues across Wales.

The CoP is one of a series of related CoPs designed to promote ongoing learning related to key themes of the ‘A Healthier Wales’ Transformation programme and the Regional Integration Fund (RIF) programme from 2022–27, and to contribute towards the RIF’s aim of developing new national Models of Integrated Care.

To read more about the work of the CoP see the document below. The next meeting is 27th July. The Autumn meetings are planned for Friday 16 September 2.00pm to 4.30pm and Tuesday 25 October 9.30am to 12.00noon
