"I'm researching models for the payment of social prescribing delivery. Specifically, I am trying to determine if there are models being used where a social prescribing link worker provides payment per each referral to the service provider. If so, I'm keen to know what the size of the payments are; whether (and how) they vary according to the service; and how these are administered. It would also be useful to know if the system (if there is one) works well and any problems related to it.
