Advance Care Planning (ACP) empowers people to think about, record and share their wishes about their future care. It helps to provide person centred care and engaging with it has been shown to provide better health care outcomes for patients, public and their loved ones.
As a result of the enthusiasm for the project we need more champions to help embed ACP across all care and support specialisms and communities in Powys. Nurses, Social Workers, Social Carers, HCSW’s, Medical Staff, Administrative Staff, Allied Health Professionals, Third Sector Volunteers and volunteers from the public are all very welcome to become ACP champions. Champions receive full training in supporting people to complete the document, signposting to further help, and promoting ACP.
Our next training is on Tuesday 12th November 2019 in Training Room 1, (Opposite Library), Bronllys Hospital, Bronllys, Brecon, LD3 0LS from 10am until 3.30pm.
(Refreshments provided, bring your own lunch!)
