As needs change over time, effective coordination between healthcare professionals, support organisations and community services becomes increasingly important.
Sarah Wilson, Powys MND Care Coordinator will be working in collaboration with the Bracken Trust as they begin accepting referrals for people living with MND. This partnership will help ensure that people affected by MND in Powys have access to a wider range of support and
resources, delivered in a coordinated and compassionate way.
