Cancer Community Connector - Improving the Cancer Journey in Powys
In partnership with Macmillan, PAVO has a dedicated Community Connector who supports people affected by cancer.
What We Offer
Our Macmillan-funded Cancer Community Connector offers a supportive, personalised conversation for those diagnosed with cancer, whether recently diagnosed or living with cancer, a family member, or unpaid carer. The service is available for people aged 18+, who are residents of Powys and living with, or affected by cancer.
A Macmillan Cancer Support holistic needs assessment is used to guide the conversation. Through this supportive conversation, people can discuss what matters to them and any concerns they have, they will receive guidance and signposting to appropriate local support and services through a person-centred care plan which could include:
- Support groups
- Financial advice
- Wellbeing resources
- Practical support
- Specialist services
