- Make sustainable, long-term changes to their health and wellbeing
- Create a shared agreement, with clear steps, mapping goals and milestones
- Focus on their strengths through a “What Matters” conversation
- Build strong circles of support to live and age well
- Access help and activities available in their community

How we support you
Our social prescribers support people until they reach their goals and feel confident managing their health and wellbeing independently. Examples could include:
- Direct support to join a local choir or singing group to support lung health
- Helping people connect with groups that share their interests so they can build their circles of support
- Introducing people to community groups, such as Versus Arthritis for support with managing joint pain
Who Do Our Social Prescribers Work With?
Our social prescribers support people aged 50–75 living in Powys whose health and wellbeing are affected by one or more of the following conditions:
- Musculoskeletal
- Heart and circulatory
- Respiratory
- Endocrine and metabolic
- Mental health

You can refer yourself or someone else to our Community Wellbeing team. They’ll review the information you share and connect you with the most suitable service.
