What is Social Prescribing?

How we support you

Our social prescribers support people until they reach their goals and feel confident managing their health and wellbeing independently. Examples could include:

  • Direct support to join a local choir or singing group to support lung health
  • Helping people connect with groups that share their interests so they can build their support networks. 
  • Introducing people to community groups, such as Versus Arthritis for support with managing joint pain

Who Do Our Social Prescribers Work With?

Our social prescribers support people aged 50–75 living in Powys and have a long-term condition affecting your:

  • Muscles or joints
  • Heart or circulation
  • Breathing
  • Hormones or metabolism
  • Mental health

How to Access Support

You can refer yourself or someone else to our Community Wellbeing team. They’ll review the information you share and connect you with the most suitable service.
