A More Joined-Up Approach to Community Wellbeing
Our Community Wellbeing Team has recently been remodelled to deliver a more coordinated service for people across Powys.
What’s Changed
Previously, support was provided by two separate services – the Powys Befriending Service and Community Connectors.
Now, these services have come together – along with our Social Prescribers – to form one integrated Community Wellbeing Team.
How the New Team Works
The new team works regionally in North, Mid, and South Powys, making it easier for people to find the right support quickly and easily – Community Wellbeing team
We’ve also introduced a single referral gateway for all enquiries, connecting people with the most suitable service and making access to support smoother than ever.
This new approach places a stronger focus on social prescribing, in alignment with the Social Model for Health and Wellbeing.
