Advance Care Planning (ACP) empowers people to think about, record and share their wishes about their future care and engaging with it has been shown to provide better care outcomes for the person with an ACP, and their loved ones. Honest conversations, informed choices, and the involvement of family and carers allows for coordinated care based on the values, beliefs and wishes of the person in a crisis, in the event of being unable to speak for themselves or at the end of their life.
