Changes to Brecon Mind Social Prescribing Services.

Brecon and District Mind now have a new link worker in post with more capacity for support in Brecon, Crickhowell, Hay & Talgarth and surounding areas.

As of the start of September Berni Laurie-Chiswell is joing the Brecon Mind team.  Berni and Rhiannon will be sharing the caseload for all three medical practices (Brecon, Crickhowell and Haygarth) and covering four days a week between them.

People living within Brecon, Crickhowell, Hay and Talgarth and the surrounding areas can access the support service. Whether their anxieties, concerns and worries are around family relationships, housing, financial matters, stress and panic attacks, depression, loneliness and general Covid-19 lock down concerns for themselves or others, Brecon Mind can help.

Community and voluntary groups, organisations and agencies can refer to a Social Prescribing link worker either by phone or email or by sending the attached referral form. All they need is the individual’s permission to contact Brecon Mind.

Alternatively, an individual can self-refer by either contacting either Bernadette or Rhiannon directly or phoning through to the main office 01874 611529 where a member of staff will take their details and forward them on.
