Powys Palliative Care team want the project to make the most difference possible, so for the next month, they are asking for your experiences of End of Life Care in Powys.
Only by understanding peoples’ experiences of these often-difficult times, can they make sure they are providing the support people want and need.
While the project is focused first on Community Hospitals, all views and experiences are welcome.
The survey takes about 10 minutes, so please complete it and share it with anyone who has experienced bereavement in Powys in the last ten years, who may want to help. The survey is open until 14th February 2021.
With End of Life care, we only have ‘one chance to get it right’ and your feedback will help to give the best support possible.
