Pathfinder - resources for continence in young children

As part of the Early Years Integration Transformation Programme (PATHFINDER) developing a more joined up responsive approach for support for children ages 0-7. The Project was fortunate to have Paediatric Continence Nurse Specialist Jennifer Walsh undertake a mini pilot of support within the Rhayader school community. This mini pilot showed the need for more information, support and resources for supporting children’s toileting needs .

As part of the project Pathfinder has complied 3 Resource Bags of Toys/Books to help Teams from across all services best support children and their families with any toileting/continence needs. The bags and resources are for use on a loan out system and could be used by teams within PTHB, PCC, Education and Third Sector. If Team members wish to loan out an item to a family also, this is possible. A full detailed explanation of the resources and contacts and borrowing sheets are attached to this mail. Each bag has a folder with all the paper work/ explanations in as well.
