The Community Connector will provide front line support within the delivery of a community based health and well-being support services, which takes a whole system approach to “what matters” to people.
The post will work within a preventative and early intervention service framework. The role will provide duty support to adults who contact the service to access local community services and activities, as well as services provided by the public, private and third sectors by “brokering” their access to these services and working with the individual to find bespoke solutions to meet their personal needs.
The post holder will develop detailed knowledge of the communities and services within the locality in which they operate; using this knowledge to develop solutions to meet the needs of individuals in need of support. As well as developing a good general knowledge of Powys wide services and support.
The post will support people to access the right support, first time, in the right way; and seeks to empower people to build their own networks of support within their community.
