Overview

The post holder will support adults to access local community services and activities provided by the public, private and third sectors, working with the individual to find bespoke solutions to meet their needs. They will support people to access the right support, first time, in the right way and seek to empower them to build their own networks of support within their community. The post holder will facilitate a locality network to connect and inform others of services to support health and wellbeing within the area. 

This position has a requirement for an Enhanced DBS Check.

Job Purpose

The Community Connector will provide front line support within the delivery of a community based health and well-being support services, which takes a whole system approach to “what matters” to people.

Key Responsibilities

The post will work within a preventative and early intervention service framework. The role will provide duty support to adults who contact the service to access local community services and activities, as well as services provided by the public, private and third sectors by “brokering” their access to these services and working with the individual to find bespoke solutions to meet their personal needs.

The post holder will develop detailed knowledge of the communities and services within the locality in which they operate; using this knowledge to develop solutions to meet the needs of individuals in need of support. As well as developing a good general knowledge of Powys wide services and support.

The post will support people to access the right support, first time, in the right way; and seeks to empower people to build their own networks of support within their community.

 

The post will work with the Third Sector Development Team to ensure joined-up provision of community based support for individuals and groups. 


Main Duties

  • Assist individuals, their families and carers to utilise personal and local community networks to develop practical solutions to support their health and wellbeing. This will be through telephone or face to face contact with client/patient/carer/professionals to ascertain needs.
  • Assist/provide support to the client to enable them to navigate and access the support and resources they need.
  • Provide Community Connector duty cover remotely either within ASSIST or with the team as and when required. ASSIST is a single point of access for adults services across Powys. This will involve taking calls from the public, clients, statutory and third sector services; providing initial information/support where possible; allocating to Community Connector in relevant area or according to caseload; dealing with enquiries via email.
  • Build effective partnerships and working relationships with community and statutory services including Police, GP’s, community organisations, Health, Adult Social Care and family services to establish early identification of and effective responses to identified need including safety and safeguarding. 
  • Facilitate a locality network to connect individuals, community groups, third sector organisations, statutory partners, professionals and business together to share good practice, local information and identify gaps in provision within the area to improve health and wellbeing. 
  • Contribute to and represent the Third sector within multidisciplinary discussions with Health and Social Care professionals and attend Multi-Disciplinary Team (MDT) meetings in local area(s). This may include Virtual Wards, Hospital MDT’s, Integrated Health and Social Care Teams, GP Surgery teams.
  • Develop and maintain useful contacts and relationships by having a clear understanding of local community strengths, resources, connections, gaps and opportunities to support the purpose of the service and people it serves.
  • Facilitate interaction with all local service providers and enable real and virtual teams to be brought in to support the individual, where needed and appropriate.
  • Promote the Community Connector service and infoengine.wales, encourage and advise third sector organisations and local groups on how they can access support to register with infoengine,
  • Identify and report gaps in service provision within the communities in which the post holder operates; working with colleagues in PAVO, Powys County Council, Powys teaching Health Board, the Third sector and other agencies to help initiate the creation of new support arrangements or services to meet the identified local community needs.
  • Maintain accurate records to ensure the service is able to gather and analyse data to support ongoing needs assessment, measurement of outcomes, service evaluation and the monitoring of the efficiency and effectiveness of the service.
  • Organise and maintain administrative and personal records, data sharing within the Wales Accord for Sharing of Personal Information (WASPI) protocols.
This position has a requirement for an Enhanced DBS Check; the position is classed as regulated activity under the Safeguarding of Vulnerable Groups Act 2006 as amended by the Protection of Freedoms Act 2012 and will be subject to a check of the list of those people barred from working with vulnerable adults.
The job description covers the main duties and responsibilities of the post but other duties commensurate with this role will also be undertaken by the post holder. PAVO aims to provide a service that is responsive to the needs of its membership and service users and it is therefore necessary that staff respond to changing requirements. Required Knowledge and Experience

  • Experience of working alongside and supporting individuals and families from a variety of backgrounds and varying needs, working within multi-agency partnerships
  • An understanding of and experience of delivering a person centred service, supporting people to plan and meet their own needs for the future
  • Good knowledge of communities within Powys and resources available that can support the delivery of the service, or where to find this information
  • Excellent knowledge of systems including WCCIS, email and VPN systems
  • Knowledge of relevant legislation (Social Services and Wellbeing (Wales) Act, Wellbeing of Future Generations Act, )
  • Understanding confidentiality, maintaining confidential records, data protection and WASPI
  • Understanding of Safeguarding
  • Understanding of and commitment to Equality and Diversity

  • Ability to travel around rural Powys
  • Welsh Language skills competency level 1 – ‘I can pronounce Welsh personal and place-names correctly, and I can give and respond to basic greetings on the telephone or in person.’

Probationary Period

The appointment is subject to the satisfactory completion of a period of probationary service of 3 months.

Travel

PAVO pays all relevant travel at the current agreed mileage rate.

20 Day holiday per year (25 days pro rata)

Pension Scheme

A contribution equal to 6% of salary will be made available to the PAVO Stakeholders Pension or to a Private Pension Scheme
