If someone is unable to attend an appointment at a mass vaccination centre and would like to request an alternative (e.g. via GP practice) or cancel completely (e.g. because they do not wish to have the vaccine) they can use the following web form:
This form can be completed by the individual themselves, or by someone on their behalf.
We do still encourage everyone who CAN attend an appointment at a mass vaccination centre to do so, as this will help us ensure that as many people as possible are vaccinated as quickly as possible. However, clearly an appointment at a mass vaccination centre is not an option for some, particularly given our rural geography, and this form provides them with another route to contact us.
If you are aware of someone aged over 80 who is registered with a GP in Powys and who has not yet received an invitation for COVID-19 vaccination you can use the form here to notify our team:
This form can be completed by the individual themselves, or by someone on their behalf.
If you are aware of someone aged 80 or over who is registered with a GP outside Powys and who has not yet received an invitation for COVID-19 vaccination, there is information about the COVID-19 vaccination programmes in neighbouring health boards on our website (we have asked neighbouring health boards in Wales and neighbouring CCGs in England to provide more detailed information so that we can advise Powys residents appropriately, so that everyone has access to the vaccine):

In addition, there is now a dedicated page for each Priority Group to provide information about the current status and when people may expect to be contacted. These pages will continue to updated as the programme progressed into February and on to the Spring. These are all available from the menu on our COVID-19 vaccination page at https://pthb.nhs.wales/coronavirus/covid-19-vaccination/, but the direct links are provided below for convenience:
