They have a survey which can be completed by people who have been diagnosed with dementia and a separate survey which can be completed by relatives or carers of someone living with dementia

The surveys are available online and also in paper format and are attached to this article for your information.  Surveys are also  available in Welsh. The survey is planned to run until the end of February.
