Model of Care
The model captures the feedback and experiences from a wide range of stakeholders, including residents across all ages, harder to reach groups, staff, and children about what works well now, and what they would like to see improved in the future at home, community, region, and out of county.
These views have been captured in a North Powys Wellbeing Engagement report, which has supported the development of the draft ‘Model of Care’ and a supporting ‘Case for Change’ document. This aims to ensure our residents’ voice is at the heart of a new model of care through a co-production approach.
During the next phase of engagement, from now until the end of January, the PTHB inivite you to share your views on the draft ‘Model of Care’.
They would like to hear your views on the future delivery model of health and care services at home, in your community, wider region of north Powys, and out of county, by completing a short survey.
