My Life My Wishes - Advanced Care Planning

Due to the seriousness of this outbreak it is now very important to start the conversation with the communities we support and for them to consider where they would wish to be cared for and what is important to them in case they become severely unwell with Corona Virus.

Now more than ever it is such an important time for your teams to promote and initiate conversations with your community clients and their families to consider where they would wish to be cared for and what is important for them and their families, in case they become seriously unwell with Corona Virus. Is being at home with family and friends important to them or being cared for in a clinical setting, even if it meant your loved ones could not visit?

Please see the new document attached which can be used along with the PTHB My Life, My Wishes Advance Care Plan and guidance document (see link below) or the attached document can be the stand-alone statement.

My Life, My Wishes documents:

If someone has clear opinions about treatments or interventions that they do not want, they can formalise their wishes by completing a legally binding Advance Decision to Refuse Treatment (ADRT). Specific details are described in the My Life, My Wishes guidance booklet and the link below is included within the Covid-19 Advance statement attached.
