The post holder will support adults to access local community services and activities provided by the public, private and third sectors, working with the individual to find bespoke solutions to meet their needs.  They will support people to access the right support, first time, in the right way and seek to empower them to build their own networks of support within their community. The postholder  will facilitate a locality network to connect and inform others of services to support health and wellbeing within the area. 

This position has a requirement for an Enhanced DBS Check.

Salary:- £21,580 per annum  (£26,975 pro rata) funded until 31 March 2023 with possibility of extension
