PURSH SUPPORT WORKER, SOUTH BRECON POWYS

Job Purpose:
Under the direction of the Regional Manager (Powys), Hafal Crossroads operates PURSH - Powys Urgent Response Service at Home. The purpose of PURSH is to prevent avoidable admissions of Powys residents to hospital through the provision of a short-term support and health care within peoples' own homes, in emergency situations when hospital admission is deemed inappropriate, reducing the number of avoidable admissions of Powys residents and ensuring the right care, at the right time as locally as possible. In line with our commitment to principles of empowerment, self-management, wellbeing and recovery and ensuring compliance with internal and external quality standards inspection and regulation (including CSSIW).

FLEXIBLE HOURS

About this job:
The service will be delivered in people's own homes and will respond flexibly to the assessed needs of the referred individual through the provision of tailored packages of care - in some instances for 24 hours per day during the urgent response service period.
Referral to the service will be via GP or virtual ward staff assessment (during the normal working day) and Shropdoc assessment (during the out-of-hours periods). Referrals are received directly from the Care Coordination Centre Shrewsbury (CCC) to the dedicated contact line operated by Hafal Crossroads.
3 PURSH Support workers are on call at all times, working within their designated area of either North, Mid or South Powys according to their location.

The PURSH service will commence the required health care support within a maximum of 2 hours of the referral being received.

Job Type: Part-time

Salary: £7.83 to £10.30 /hour
