Health & Social Care Integrated Leadership Board

This group provides strategic oversight of the One Powys Transformation Programme that seeks to develop Integrated health and social care services. Carl Cooper represents PAVO and the Third Sector on this group.

Integrated Care Pathway for Older People

The Integrated Care Pathway for Older People (ICPOP) has responsibility for monitoring the development of the work programme set out in the statement of Intent.

Thematic groups oversee the implementation of support for Carers, Physical Disability & Sensory Loss, Learning Disabilities, Older People and Mental Health.

The Third Sector perspective is fed into ICPOP and the thematic groups by PAVO and is informed by the Health & Social Care Network and by the Partnership Forums

Promoting Health and Well-Being of People in Powys

Powys Public Health Team seeks to work in partnership to improve the health of people in Powys through the promotion of Healthy Lifestyles and prevention of illness. Third Sector contributes to the following workstreams:
