"When an individual suffers a stroke, they need rapid assessment and the best
possible diagnosis, treatment and on-going support. Alongside excellent treatment a
focus is required on actions which prevent stroke occurring in the first place. The
Stroke Delivery Plan sets out my expectations of NHS Wales, working with its
partners, in tackling stroke up to 2016.

Key priority is to reduce inequalities in health across the life course in Powys through the following actions:

Pregnancy

Develop a tobacco control action plan, ensuring that all pregnant women have access to appropriate and effective smoking cessation services.

Early years

Develop a Powys Healthy Pre-Schools Scheme to promote all aspects of health and give young children the best start in life.

School aged children

Support the pilot Community Alcohol Project, to promote a partnership approach to protecting children from alcohol related harm.

Adults

Develop a healthy weight strategy with partner organisations.

Older people

Ensure that healthy ageing is everybody's business by working with partners to include healthy lifestyles in strategies and services being developed for older people.

Elderly

To work with partners in the health sector, statutory sector and Voluntary and Community Organisations to increase uptake of flu vaccination.

Wellbeing Assessment

Wellbeing Assessment of the Community Weight Management Programme run in the valley's north of Bridgend. The programme itself was successful in producing a significant change in weight amongst the participants. It was commissioned by Bridgend Public Health team to look at some of the issues that supported people's participation in the programme from a wellbeing perspective - to try and understand the individual in the context of their community as a means to make the programme even more successful and tweak where necessary.

Powys Participation Strategy and Action Plan

This Participation Strategy has been produced by Powys Local Service Board (LSB) utilising support from the Making the Connections Fund.

This strategy aims to bring together, in a coherent form, the various mechanisms through which the LSB will support and champion citizen and community participation. It is based on a simple framework of Individual (including family unit), Interest and Place.

Bridging the Gap -  Addressing the challenge to fully utilise the third sector contribution in the management of complex care.

This document has been developed by the Continuing NHS Healthcare National Programme for the attention of Local Health Boards, local authorities and third sector organisations across Wales. It demonstrates the range of third sector services and schemes in place across Wales that complement and support statutory services in maximising all opportunities for independent living.  All organisations are asked to consider this report and the related action plan as part of their planning and service delivery arrangements.
